Mr. P.

“The Love of His Life”. In the year following the death of his wife,
Mr. P. had several medical problems including heart problems and the
new onset of Diabetes.

Mrs. R. is in Mid-stage Alzheimer’s Disease. Her husband is medically
stable and his mental status is good. Mr. & Mrs. R. started care
management services when Mr. R. realized he needed additional care
in the home and respite for himself from the 24 hour responsibility
of caring for his wife. Their four children are very supportive but
all live out of state.
